How it works

It asks about the things people forget

Most people describe their good days, and leave out the aids they use, the help they get and how long things take. After each section, Dearform shows you what you have not mentioned yet. You choose whether to add it.

Is it really free?

Everything in Dearform is free to use. If it helped you, you can pay what you can to help cover the cost of running it. Paying does not unlock anything.

Where do my answers go?

Your answers are saved on your device and never sent to us — we cannot see them. There is no account, and we never ask for your name, address or National Insurance number. Read our privacy notice.

Who is it for?

People in England, Wales and Northern Ireland claiming Personal Independence Payment (PIP), and anyone helping them fill in the form.

I live in Scotland. Can I use it?

In Scotland, PIP has been replaced by Adult Disability Payment, from Social Security Scotland. It uses a different form, and Dearform does not cover it.

What is Dearform not?
  • It is not part of DWP or the government.
  • It does not send anything to DWP. You send your own form.
  • It cannot tell you whether you will get PIP. DWP decides that.
  • It is not legal, medical or benefits advice.
How do I put the answers on my form?
  1. Fill in your form as it asks, including the tick boxes, your details, the health professionals in Question 2, and the declaration, which you sign.
  2. For each question, put your answer in the box on the form in whichever way suits you: copy it out in full, or write a short summary and add "see extra sheet". On the online form, you can paste it in.
  3. If you send printed answers as extra sheets, write your name and reference number at the top of each page, as the form asks. The reference number is on the letter that came with your form, and is usually your National Insurance number.
  4. Check every word before you send anything, and keep a copy.
