These terms apply when you use Dearform. By using it, you agree to them. They do not affect your legal rights.

What Dearform does

Dearform helps you write answers for the PIP2 form, “How your disability affects you”, from what you tell it. It writes up only what you said, and shows you what you have not mentioned. You can change every word.

Dearform does not:

• work for, or on behalf of, DWP or the government
• send anything to DWP — you send your own form
• decide, or predict, whether you will get PIP or how much — DWP decides your claim
• give legal, medical or benefits advice

Your answers are your responsibility

You decide what goes on your form, and you sign it. Check every word before you send anything. Only describe what is true for you. Knowingly giving false information on a benefit claim is against the law. Do not use Dearform to write anything that is not true.

Keeping your answers

Your answers are saved only in your browser, on your device (see our privacy notice). We do not have a copy, so we cannot get them back if they are lost. Browsers can clear saved data, for example when a phone runs low on space, so copy or download your answers once you have finished, and keep a copy of what you send to DWP.

It is free, and paying is optional

Everything in Dearform is free to use. If you choose to pay what you can, the payment helps cover the cost of running Dearform. It does not unlock or change anything, and it is not a donation to a charity. Payments are taken by Stripe, and Stripe’s own terms also apply to the payment.

Refunds

If you paid by mistake, or paid more than you meant to, contact us and we will refund you in full. Refunds normally go back the way you paid. Email laivehealth@gmail.com.

Keeping Dearform accurate

Dearform’s questions follow the PIP rules in the law and the PIP2 form as we last checked them. We work to keep it accurate, but the rules and the form can change, and we cannot promise Dearform is always up to date or free of mistakes. We may change Dearform, or stop running it.

Our responsibility to you

We are not responsible for DWP’s decision on your claim, or for what you choose to put on your form. We are not responsible for loss that was not a foreseeable result of something we did wrong. Nothing in these terms limits our responsibility for death or personal injury caused by our negligence, for fraud, or for anything else the law does not allow us to limit.

Using Dearform fairly

Please do not try to disrupt Dearform, overload it, or get into parts of it you are not meant to.

The law that applies

These terms are governed by the law of England and Wales. If you live in Northern Ireland, you can also bring a claim in the courts of Northern Ireland, and the law of Northern Ireland still protects you.
