Who we are

Dearform is responsible for the personal information described here — in data protection law, the “controller”. Email laivehealth@gmail.com.

Your answers

Everything you answer, and every written answer Dearform makes from it, is saved in your browser’s storage on the device you are using. Your written answers and the PDF are made on your device too. None of it is sent to us, to our hosting provider, to our analytics provider or to anyone else. That means we do not handle your health information at all.

Before you start, we ask you to agree to your answers being saved on your device. We keep a note of that agreement on your device too, beside your answers.

Because we never have your answers, we cannot see, correct, recover or delete them for you — you are in control of them. You can remove them at any time with Delete my answers, or by clearing this site’s data in your browser settings. Anyone else who uses the same device and browser could see them, so if you share a device, delete your answers when you have finished. Anything you copy or download is yours to keep or delete.

If you pay

Paying is optional. If you choose to, you pay on a page run by Stripe, our payment provider. Stripe collects what it needs to take the payment, such as your email address, the name on your card and your card details. We can see the amount, your email address, the name on your card, your billing country and postcode, and the last four digits of your card. We never see your full card number.

We use this to take the payment you asked for, to send your receipt (Stripe sends it for us), to refund you if you ask, and to keep the business records the law requires. Our lawful bases are that you asked us to take the payment (contract) and that we must keep tax records (legal obligation). We keep payment records for as long as the law requires us to keep business records: at least 5 years after the 31 January tax return deadline for the year of the payment. Stripe also keeps its own records of payments, for its own legal duties such as preventing fraud; its privacy notice explains how long.

When you start a payment, we set one cookie (see “Cookies and storage” below) so that you are never charged twice and we can confirm when your payment has gone through.

Counting visits

We use PostHog to count how many people visit our pages, and how many start and finish each section of the form, so we can see where people get stuck and fix it. It is set up to store data in the EU (Frankfurt, Germany). It does not use cookies and does not keep anything on your device. To count visitors without cookies, it uses your IP address and browser details to make a code that changes every day; it is set not to store your IP address.

What it receives:

• the pages you visit and their titles, without anything after a “?” in the web address
• the website you came from, if any, without the page
• the name of the campaign or post that sent you, if the link you followed was tagged with one
• how long you spent on the page before, and how far down it you scrolled
• your type of browser, device and operating system, your screen size and time zone
• when a section is started, when a section is finished, and when an answer is copied — which section, and nothing else
• when the PDF is downloaded — how many sections it held, and nothing else
• the amount of a payment, with nothing about who made it

It never receives anything you typed or ticked. Our lawful basis is our legitimate interest in knowing whether Dearform works for the people using it. You can object at any time by contacting us, or block it with your browser’s tracker or content blocker — Dearform works the same without it.

Hosting

Dearform is hosted by Vercel. Like any website, its servers see your IP address, your type of browser and the pages you ask for, and keep this in logs for a short time to run the site and keep it secure. Our lawful basis is our legitimate interest in keeping Dearform running and safe.

If you email us

We use your email address and message to reply to you. We delete them within 12 months of the last reply, unless they are about a payment, in which case we keep them with our payment records.

Where your information goes

Stripe and Vercel are based in the United States and may handle information there. PostHog is also a US company, though it stores Dearform’s data in the EU. Where information leaves the UK, it is protected by safeguards that UK data protection law recognises. We do not sell your information, share it for advertising, or use it to make automated decisions about you.

Cookies and storage

• Your answers (browser storage, “dearform.answers.v1”): what you have answered, which sections you have finished, and any changes you made to your written answers. Kept until you delete them.
• Your agreement (browser storage, “dearform.consent.v1”): that you agreed to your answers being saved on this device. Deleted with your answers.
• Your payment (cookie, “df_order”): set only if you start a payment. A random code that links this browser to your payment. Kept for 60 days.

All three are needed for things you have asked Dearform to do. There are no advertising or analytics cookies.

Your rights

You have the right to ask for a copy of the personal information we hold about you, and to ask us to correct it, delete it, stop or limit using it, or send it to you or someone else. You can object to how we use it. Contact us to do any of these; we will reply within one month. Remember that we do not hold your answers — only you do.

If you are unhappy with how we have handled your information, please tell us first. You can also complain to the Information Commissioner’s Office (ICO): ico.org.uk/make-a-complaint, or call 0303 123 1113.

Changes to this notice

If we change how we handle information, we will update this page and the date at the top.
